Part-Time CAL RN 1

We are recruiting CAL RNs for our Population Health team.

Role: Provide on-demand telephone-based care management services (24/7 Nurse Advice Line).

• Assess caller symptoms/concerns to determine care urgency and type.
• Refer or schedule appointments with providers.
• Provide health information and advice to callers.
• Reduce unnecessary clinic and emergency department visits.
• Coordinate care across the healthcare delivery system.
• Operate in a team-based model.

Job Duties:

1. Handle inbound patient communications.
2. Conduct outbound communications for follow-up and care coordination.
3. Consult and coordinate with team members to assess, plan, implement, and evaluate patient care plans; make appropriate referrals; provide follow-up.
4. Assist in finding providers, community resources, and care solutions; coordinate priority appointments.
5. Record member data in Privia's web-based medical record system.
6. Research information online and in Privia's databases.
7. Provide health information, coaching, and critical thinking to members.
8. Other care management activities as needed.

Qualifications:

• Registered Nurse (RN) with current resident compact licensure.
• Bachelor's degree.
• Minimum 3-5 years of recent clinical experience.
• Experience in a call center, triage, consulting, or similar environment.
• Strong computer skills; internet savvy.
• Clear, confident communication and listening skills.
• Self-motivated and self-disciplined.
• Exceptional customer service skills.
• Bilingual (Spanish, Korean, Vietnamese, or Farsi preferred).
• Detailed-oriented, organized, and able to work in a fast-paced environment.
• Must comply with HIPAA rules.

Schedule: Primarily nights, weeknights, and holidays (9pm-9am EDT) with flexible, alternating shifts (2, 4, 8, 6, 10 & 12 hours).

Must maintain at least 16 hours of night/weekend coverage/week (64 hours/month).

Technical Requirements (remote): Minimum 5 Mbps download speed and 3 Mbps upload speed.

Salary: $25-30/hr.
