Part-Time CAL RN 1

We are actively recruiting for an CAL RN's to join our rapidly growing Population Health team.

Primary role: Provide on-demand telephone-based care management services through a 24/7 Nurse Advice Line (telephone triage). Assess callers' symptoms/concerns to determine urgency and type of care needed; refer/schedule appointments with providers; give health information & advice.

Goal: Reduce unnecessary clinic/emergency department visits; provide self-care & symptom management information; coordinate care.

Team-based model: Acts as an extension of the primary care provider.

1. Handles inbound communications from patients seeking information about symptoms or care concerns.
2. Conducts outbound communications for follow-up and care coordination.
3. Consults and coordinates with internal/external team members to assess, plan, implement, and evaluate patient care plans; make referrals and provide follow-up.
4. Assists with finding appropriate providers, community resources, care solutions, and coordinates priority appointments.
5. Records member data in Privia's web-based medical record system and associated EMRs/health portals.
6. Researches information online and in Privia's internal knowledge databases.
7. Provides health information, coaching, and critical thinking skills to assist members with medical and wellness issues.
8. Other care management activities as needed (e.g., close "gaps in care," complex care plans).

Must comply with HIPAA rules and regulations.

Part-time role: Primarily available for nights, weeknights, and holidays (9pm-9am EDT) with flexible, alternating shifts (2, 4, 8, 6, 10 & 12 hours). Must maintain at least 16 hours of night/weekend coverage/week (or 64 hours/month). Weekends: Friday 9pm EDT - Monday 9am EDT.

Flexible time off: Available as needed.

Home office setup: Privia provides computers, high-speed internet, and other needed equipment.

Qualifications:

• Registered Nurse (RN) with current resident compact licensure in assigned state(s).
• Bachelor's degree.
• 3+ years recent clinical experience, problem-solving, & critical-thinking skills.
• Experience in a call center, triage, consulting, or similar environment.
• Disease Management, Case Management, Utilization Review, or Wellness experience preferred.
• Excellent customer service skills.
• Strong computer skills and internet savvy.
• Clear, confident communication & listening skills.
• Self-motivated & self-disciplined.
• Willingness to do what it takes to prioritize patients.
• Able to thrive in a quiet, secure home office environment.
• Detailed-oriented, organized, and capable of working in fast-paced environments.
• Bilingual in Spanish, Korean, Vietnamese, or Farsi preferred.

Technical requirements (remote only): 5 Mbps download speed, 3 Mbps upload speed.

Compensation: $25-$30/hour (base pay).
