Intake and Assessment Officer
Join the Community Health & Wellbeing team at Hume City Council, one of Australia’s fastest growing and culturally diverse councils.
What you’ll be doing:
- Provide tailored assistance to eligible clients, including individuals from Aboriginal and Torres Strait Islander (ATSI) and Culturally and Linguistically Diverse (CALD) backgrounds.
- Conduct individualised assessments of clients to create effective, person-centred care plans.
- Identify and address gaps in access to services for SAH, HACC PYP, and CHSP target groups.
- Evaluate the effectiveness of the ADS intake.
- Facilitate access to available services for target groups.
- Actively promote effective teamwork, communication, and collaboration across teams.
- Seek out learning opportunities to enhance skills and knowledge.
- Stay informed of industry trends and best practices.
Skills and Experience:
- Demonstrated knowledge of the Aged Care Act and relevant regulations.
- Highly developed assessment skills.
- Ability to liaise with medical, allied health, and community professionals.
- Highly developed written and verbal communication skills.
- Ability to communicate effectively with diverse groups.
- Ability to maintain administrative records.
- Experience working with diverse client groups.
Required:
- Degree or diploma in a relevant field or equivalent experience.
- Current driver’s licence.
