What you’ll be doing:

• Provide tailored assistance to eligible clients, including individuals from Aboriginal and Torres Strait Islander (ATSI) and Culturally and Linguistically Diverse (CALD) backgrounds.
• Conduct individualised assessments of clients to create effective, person-centred care plans.
• Identify and address gaps in access to services for SAH, HACC PYP, and CHSP target groups.
• Evaluate the effectiveness of the ADS intake.
• Facilitate access to available services for target groups.
• Actively promote effective teamwork, communication, and collaboration across teams.
• Seek out learning opportunities to enhance skills and knowledge.
• Stay informed of industry trends and best practices.

Skills and Experience:

• Demonstrated knowledge of the Aged Care Act and relevant regulations.
• Highly developed assessment skills.
• Ability to liaise with medical, allied health, and community professionals.
• Highly developed written and verbal communication skills.
• Ability to communicate effectively with diverse groups.
• Ability to maintain administrative records.
• Experience working with diverse client groups.

Required:

• Degree or diploma in a relevant field or equivalent experience.
• Current driver’s licence.
