Part-Time Intake and Assessment Officer

Join the Community Health & Wellbeing team at Hume City Council.

Collaborate with clients, families, and service providers to develop personalised care plans.

What you’ll be doing:

• Provide tailored assistance to eligible clients from ATSI and CALD backgrounds.
• Conduct individualised assessments to create effective, person-centred care plans.
• Identify and address gaps in access to services for SAH, HACC PYP, and CHSP target groups.
• Evaluate and assess the effectiveness of the ADS intake.
• Facilitate access to available services for target groups.
• Actively promote effective teamwork and communication.
• Take ownership of personal and professional development.
• Stay informed of industry trends and best practices.

The skills and experience you’ll bring to the role:

• Demonstrated knowledge of the Aged Care Act and relevant regulations.
• Highly developed assessment skills.
• Ability to liaise with medical and community professionals.
• Strong written and verbal communication skills.
• Ability to work with diverse client groups.

Required Certifications & Qualifications:

• Degree or diploma in a relevant field or equivalent experience.
• Current driver’s licence.
