The Clinical Nurse Coordinator (CNC) serves as a clinical case manager for patients and caregivers. The CNC is a clinical resource for patients and families, and provides education on specific disease states, lifestyle impact and lifecycle/age specific considerations. The CNC will work with other team members to help patients be successful with their prescribed therapies. The CNC helps to determine which patients need assistance in improving medication adherence and devises strategies to accomplish this. Establishing regular contact with the patients is essential to the position.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provide overall clinical case management services to patients and caregivers, develop 1:1 relationships
- Ensure all assigned patients are contacted on a regular basis, adapt frequency based on at risk patient assessments
- Assist patients and caregivers in the understanding of care and treatment plans
- Assist patients and caregivers in overcoming identified barriers and connect to available manufacturer, and EVERSANA community resources
- Collaborate with pharmacists and members of the care team to improve patient adherence to prescribed treatment plan
- Delivery of superior nursing service and clinical education
- All other duties as assigned
EXPECTATIONS OF THE JOB:
- Interface between program patients, payers, and health care providers to ensure quality nursing services are delivered to patients.
- Follow up with program patients to proactively support patient care
- Perform patient/family education about disease state, management/plan of care, medication use and side effects, lifestyle and lifecycle considerations and resources
- Coordinate with Pharmacists, PSCs and reimbursement team to support patient medication needs
- Manage adverse events reporting according to SOP
- Provide support for recommended lifestyle modifications
- 10% travel may be required
MINIMUM KNOWLEDGE, SKILLS, AND ABILITIES:
- Licensed Registered Nurse in the state of Missouri, in good standing
- 3 years of professional nurse experience
- Analytical, creative and excellent interpersonal skills essential
- Computer proficiencies, including Microsoft office
- Ability to adapt to a fast-paced dynamic environment, makes clinical decisions, and maintains confidential information
- Strong communication skills with the ability to develop and articulate ideas and information that generate understanding
- Exhibits an upbeat attitude and genuine interest in helping patients; energizes others through his/her attitude
- Able to connect and understand the emotional aspects of patients with chronic diseases
- Proves to be a professional of high integrity and character who demonstrates ethical behavior
PREFERRED QUALIFICATIONS:
- Bachelor of Science in Nursing degree
- 5 + years relevant clinical nursing experience
- Experience working with patients with chronic diseases in an education role
