The Licensed Practical Nurse (LPN) - Remote Therapeutic Monitoring Specialist plays a critical role in the coordination and execution of Remote Therapeutic Monitoring (RTM) services, particularly those associated with CPT codes 98980 and 98981. This fully remote position involves monitoring patient data, providing timely interventions, and ensuring that patients receive optimal care through the effective use of RTM technologies. The LPN will act as a key liaison between patients, healthcare providers, and technical support teams to ensure seamless communication and care delivery.
Responsibilities:
- Patient Monitoring & Data Management:
- Continuously monitor patient data collected through remote therapeutic devices, ensuring that any significant changes are promptly reviewed and addressed.
- Analyze data trends to identify potential health issues and escalate concerns to the supervising healthcare provider as necessary.
- Ensure accurate documentation of all patient interactions and data analysis in the Electronic Health Record (EHR) system, in compliance with CPT codes 98980 and 98981.
- Patient Interaction & Education:
- Conduct regular, scheduled check-ins with patients to discuss their monitoring data, address any concerns, and provide education on device usage.
- Offer technical support and guidance to patients regarding the use of RTM devices, ensuring they understand the importance of compliance with their monitoring regimen.
- Care Coordination:
- Collaborate with physicians, nurse practitioners, and other healthcare team members to review patient data and adjust care plans accordingly.
- Coordinate follow-up actions and interventions based on patient monitoring data, ensuring timely and appropriate care adjustments.
- Act as the primary point of contact for patients regarding their RTM programs, ensuring all questions and concerns are addressed promptly.
- Billing & Documentation:
- Track and document time spent on patient monitoring and care management activities to ensure accurate billing under CPT codes 98980 and 98981.
- Assist in the preparation and submission of billing documentation, ensuring compliance with all relevant healthcare regulations and standards.
- Quality Assurance:
- Participate in quality improvement initiatives aimed at enhancing the effectiveness of RTM programs and improving patient outcomes.
- Regularly review and update RTM protocols and procedures to align with best practices and regulatory requirements.
Qualifications:
- Education & Licensure: Current LPN license in NJ, with a minimum of 2 years of clinical experience. Previous experience in telehealth or remote patient monitoring is highly desirable.
- Technical Proficiency: Strong understanding of RTM technologies, including experience with remote monitoring devices and EHR systems.
- Communication Skills: Excellent verbal and written communication skills, with the ability to effectively educate and support patients remotely.
- Organizational Skills: Highly organized, with the ability to manage multiple tasks and patient interactions simultaneously.
