The Triage Nurse (RN) assesses patients’ health problems and needs, develops and executes nursing care plans, and assists in maintenance of patient medical records. The Triage Nurse (RN) work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
Essential Functions:
- Responsible for evaluation of patients with ambulatory diagnoses as well as possible acute conditions and medication refills as appropriate. The triage nurse will work in partnership with the center’s mid-levels and PCPs.
- In consultation with the center’s mid-levels and / or PCPs the triage nurse will initiate treatment, or schedule in-center visit, if necessary, for common ambulatory diagnoses.
- Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions.
- Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.
- Resolves patient problems and needs by utilizing multidisciplinary team strategies.
- Work in other duties as assigned.

Required Qualifications
- Must be willing and able to perform the following procedures or at least be willing to be trained to do them:
- 2 to 5 years of Primary Care experience
- Good communication skills
- Works well in a team of healthcare providers.
- Licensed in the state of Kentucky
- Previous triage experience preferred
Education:
- Associates Degree or Bachelor of Science Degree in Nursing
- Nursing Diploma

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$69,800 - $96,200 per year
