Behavioral Health Care Manager role The BHCM is a key member of our collaborative care team. The role is partly clinical and partly operational, collaborating between external specialists (oncologists) and the internal care team.
Key Traits: Experienced and passionate about helping cancer patients, strong telehealth engagement skills, highly organized, adaptable, and interested in a start-up environment.
Key Responsibilities:
- Conduct initial clinical member biopsychosocial intakes, including safety screens and BH assessments (PHQ-9, GAD-7, etc.).
- Engage in follow-up visits and asynchronous interactions, including brief interventions (e.g., behavioral activation, mindfulness).
- Act as a key care coordinator, liaison between referring specialists and the practice, ensuring member/administrative questions are addressed.
- Lead weekly interdisciplinary team meetings, focusing on high-risk members and documenting psychiatric recommendations.
- Communicate regularly with the behavioral health care team (Coaches and Psychiatrist).
- Document member progress in a HIPAA-compliant system.
- Identify patients needing more intensive care and report concerns to the medical provider and psychiatrist.
- Facilitate referrals for clinically indicated services outside Cerula Care, supervised by the BHCM Lead.
Educational, Certification, and Experience Requirements:
- Bachelor's Degree in nursing, social work, or related health field.
- Experience as a CCM, CHW, PSS, or ACM is preferred.
- Minimum one year of clinical care management experience, preferably with cancer or advanced illness patients.
Location Requirements: Due to California regulations, we are not accepting contract positions from California.
Time Commitment, Salary, Start Date:
- Part-time, hourly (1099 contractor).
- Month 1-2: 10-20 hours/week
- Month 3+: 30-40 hours/week
- Hourly rate: mid-20s
- Start date: end of June 2024
