Full-Time Care Navigator, Lead

• Recruit, train, and direct a high-performing team of Care Navigators.
• Provide coaching, mentorship, and ongoing development.
• Establish performance goals and metrics.
• Play a key role in developing educational programs.
• Ensure patient resources are clear and tailored.
• Collaborate with cross-functional teams to align messaging.
• Oversee resource training in reimbursement navigation.
• Identify barriers to access and design solutions.
• Lead the launch of the Care Navigator model.
• Gather insights to continuously improve support programs.
• Partner with compliance teams to maintain program integrity.
• Develop reports providing insights into the program’s performance.

• Bachelor’s degree in Nursing, Business, or Health Sciences
• 10 years experience in Patient Support Services preferred
• Product launch experience in rare disease is highly desirable
• Strong interpersonal skills
• Direct patient experience required
