Full-Time Remote Care Coordinator

Chronic Care Staffing (CCS) is seeking a Remote Care Coordinator (CMA) to join our growing healthcare team. As a Care Coordinator, you’ll play a vital role in helping patients manage their chronic conditions.

• Conduct monthly Chronic Care Management (CCM) calls or verbal enrollments.
• Perform services such as Health Risk Assessments and Remote Patient Monitoring.
• Communicate effectively with patients and family members about chronic conditions.
• Collaborate with providers to ensure coordinated care.
• Promote adherence to personalized care plans and patient health goals.
• Support patients with medication reconciliation and education.
• Connect patients to community resources.
• Maintain compliance with CMS guidelines.

• Unencumbered active CMA or RMA certificate
• Active BLS certification
• High proficiency in EHR systems
• Recent experience in a clinical or CCM setting
• Proficiency in Google Suite
• HIPAA-compliant home office (locked door, distraction-free, two monitors, high-speed internet)
• Excellent communication skills
• Strong clinical reasoning, problem-solving, and critical thinking abilities
