Full-Time Senior Care Navigator

• Demonstrates a keen operational eye in order to execute on frequently iterated, complex workflows.
• Gain proficiency in our tools and workflows to successfully navigate our internal operations.
• Conduct outreach to understand higher risk clients’ needs.
• Support clients along their care journey via email or live chat.
• Assist in collaborative projects across departments.
• Utilize expertise to recommend appropriate providers, optimizing provider utilization and addressing concerns.
• Collaborate with providers to offer initial clinical insights.

• 2-3 years experience in Care Navigation/Coordination/Intake, specifically within mental health or healthcare.
• Master's degree in social work, clinical mental health or related area or equivalent experience.
• Experience with telephonic crisis intervention.
• Excellent verbal and written communication skills.
• Detail-oriented and organized.
• Operational experience within a fast-growth healthtech organization is ideal.
• Self-starter, proactive, and solution-oriented.
• Nimble and open to change.
• Available to work 8:00am to 5pm and/or 9:00am to 6pm PST.
