Full-Time Specialty Care Navigator - SUD

Position Overview: Full-time, remote Clinical SUD Navigator supporting individuals with substance use disorders (SUDs). Reports to the Clinical Manager of the SUD Program. Hours: Monday-Friday, 1pm-9pm EST (or equivalent).

Responsibilities:

• Conduct outbound calls and follow-ups with individuals interested in or referred for SUD treatment.
• Engage with clients using advanced clinical techniques to assess needs, provide treatment options, and make care recommendations.
• Independently coordinate appointments, referrals, and follow-up care.
• Apply psycho-education, motivational interviewing, and evidence-based practices to support client goals.
• Utilize knowledge of SUDs, ASAM levels of care, and treatments (e.g., contingency management, MAT, detox).
• Provide support, crisis intervention, and safety planning for urgent/high-risk situations.
• Maintain detailed records in an EHR.
• Identify and resolve complex issues impacting care.
• Collaborate with multi-disciplinary teams.
• Participate in staff meetings, case consultations, and training.
• Adhere to confidentiality, compliance, and ethical standards.
• Provide support and coverage to other SUD Navigators.

Qualifications:

• Mental Health Licensure required: LCSW, LMFT, LPC with 2+ years of experience in SUD clinical care.
• Proficiency in motivational interviewing, triage, ASAM criteria, and various levels of care.
• Strong listening, written, and verbal communication skills.
• Ability to work with diverse populations and embrace diversity/inclusion.
• Comfortable with computer platforms and tech tasks.
• Ability to manage multiple tasks.
• Embraces new challenges.
